Note that the following must be submitted along with this application form:

ACEN registration certificate and certificate of ACEN good standing, or for service providers other than engineering firms, they must be registered with their recognized associations, as applicable.
*Letter confirming the principal representing the company at ACEN.
Company documentation indicating shareholding and directors in the case of a PTY, or members interest in the case of a cc or trustee and beneficiary interest in the case of a trust or a partnership agreement in the case of a partnership. The documentation must be sufficient to prove majority ownership and management of the company by Namibians.
Confirmation of PI insurance and the value thereof.
CVs of all *engineers and/or technical staff indicating their work experience. The CV can be reduced but must indicate relevant experience to satisfy the requirements of years of experience before and after professional registration. The CVs must also be clear on the field of experience (i.e., materials investigation design, site supervision, project management, etc.)
List of projects conducted by the SMCEC to confirm a track record of at least three years.
Letters/Certificates of good standing with the Receiver of Revenue and Social Security Commission. These letters/certificates will not be older than one year and will be submitted yearly to update the vendor details.
At least three reference letters from clients/employers for ongoing or completed projects.
A company capacity statement.
A list of recent and relative projects currently engaged in or completed within the last five years.
Bank confirmation of good standing.
